on 35 years of experience providing case management to persons with
disabilities, Cynthia Ward founded Chrysalis Case Management when
her own family needs pointed to the need for advocacy and care planning.
Chrysalis Case Management adheres to the standards and practices
guidelines of the National Association of Professional Care Managers
and the Commission of Certification of Case Managers
Independently owned and operated, Chrysalis Case Management teams
with specialists and professionals in nursing, social work, elder
law, financial management, long term care planners and other disciplines
to ensure comprehensive assessment and appropriate care planning.

Statement of Goals
To help elders stay in their homes for as long as possible by:
- Long-range planning, to prevent crises from occurring.
- Planning financially, so that client's resources last as long as possible.
- Strengthening clients and their families to allow them to achieve the goals they have set for themselves with the assistance of the Case Manager.
- Using an interdisciplinary approach that addresses all of the client's needs as fully as possible.
- Providing assistance in creating, implementing, and monitoring a Plan of Care to ensure it continually meets the needs and desires of the client.
- Providing assistance with choosing and monitoring of alternative arrangements when remaining in the home is no longer practical or appropriate.

Chrysalis Case Management is committed to safeguarding private
and privileged material and case information. It is only with
written authorization that any information will be requested or
disclosed. If you authorize Chrysalis Case Management to request
or release your private information, the Case Manager will talk
with you about how the information will be used. It is up to the
service recipient to tell Chrysalis Case Management who the Case
Manager can share information with on your behalf.
